Positions Available: • Part-Time, Triage & On-call • Thursday 5:30 pm – 12:00 am (Triage) & Sunday 4 pm – 12:30 am (On-call visiting) Requirements: Responsibilities: • Triage patient needs via phone • Dispatch nurses/other clinicians • Obtain MD orders • Process MD Orders (medications, DME, other) • Complete all tasks assigned (Routine Visits, PRN Visits, Continuous Care, etc) • Collaborate with Interdisciplinary Team (Physician, Nursing, Social Service, and Chaplains) on a Plan of Care individualized to patient • Perform other duties as assigned Qualifications: • Bilingual in English and Spanish- required • Must be an LVN • Experience in a medical field preferred • Knowledge of medical terminology • Able to work independently with minimal supervision • Exceptional written and verbal communication skills • Pleasant phone demeanor Haven Health is committed to serving patients and their families with compassion, dignity, and grace.
