LVN at Mission Community Hospital plays a critical role in
coordinating and managing patient care to ensure optimal health
outcomes. This position involves comprehensive assessment,
planning, implementation, and evaluation of patient care plans in
collaboration with multidisciplinary healthcare teams. The Case
Manager LVN acts as a liaison between patients, families, and
healthcare providers to facilitate effective communication and
continuity of care. By monitoring patient progress and advocating
for necessary resources, this role helps reduce hospital
readmissions and supports efficient utilization of healthcare
services. Ultimately, the Case Manager LVN contributes to enhancing
patient satisfaction and quality of care within the hospital
setting. Minimum Qualifications: Valid Licensed Vocational Nurse
(LVN) license in the state of California. Minimum of 2 years of
clinical nursing experience, preferably in a hospital or healthcare
setting. Basic knowledge of case management principles and patient
care coordination. Strong understanding of medical terminology and
healthcare documentation standards. Current CPR certification.
Preferred Qualifications: Experience in case management or
discharge planning within an acute care environment. Familiarity
with electronic health record (EHR) systems and healthcare
information technology. Certification in case management (e.g.,
CCM) or related specialty. Excellent interpersonal and
communication skills to effectively interact with diverse patient
populations. Ability to work collaboratively within
multidisciplinary teams. Responsibilities: Conduct thorough patient
assessments to identify healthcare needs and develop individualized
care plans. Coordinate with physicians, nurses, social workers, and
other healthcare professionals to implement and adjust care plans
as needed. Monitor patient progress and document clinical
information accurately in medical records. Provide education and
support to patients and their families regarding treatment plans,
medications, and post-discharge care. Facilitate communication
between patients, families, and healthcare teams to ensure seamless
care transitions. Assist in discharge planning by arranging
necessary services and follow-up appointments. Advocate for patient
needs and coordinate access to community resources and support
services. Ensure compliance with hospital policies, regulatory
requirements, and quality standards. Skills: The Case Manager LVN
utilizes clinical nursing skills daily to assess patient conditions
and implement care plans effectively. Strong communication skills
are essential for educating patients and coordinating with
healthcare professionals to ensure continuity of care.
Organizational skills enable the management of multiple cases
simultaneously while maintaining accurate and timely documentation.
Problem-solving abilities are applied to address barriers to care
and facilitate resource access for patients. Additionally,
proficiency with electronic health records supports efficient
information management and compliance with healthcare
regulations.
