We are seeking an experienced and dynamic Hospice Agency Liaison to
join our team. The Hospice Liaison serves as the connecting link
between patients, families, and hospice care services, ensuring a
smooth transition and personalized care plan tailored to each
patients unique needs. This role involves educating and consulting
with healthcare providers, patients, and families about the
benefits and services provided by hospice care, facilitating a
deeper understanding and acceptance of this compassionate approach
to end-of-life care. By working closely with medical professionals
and care teams, the Hospice Liaison helps to coordinate care that
honors the patients wishes, promotes comfort, and supports the
emotional well-being of both patients and their loved ones during a
sensitive time. Through their efforts, they play an instrumental
role in making hospice care accessible and understood, ensuring
that patients receive the dignity, respect, and quality of life
they deserve. Hospice Liaison Job Duties Facilitate communication
between patients, families, and the hospice care team to ensure
understanding of the hospice philosophy and the care plan. Assess
patient eligibility for hospice care based on medical criteria and
communicate findings to healthcare providers and families. Provide
detailed explanations of hospice services and benefits to patients,
families, and referral sources, such as hospitals and physicians.
With the support of the Intake team, coordinate the initial care
plan by scheduling the first visit from the hospice team and
ensuring all necessary medical equipment and medications are in
place. Develop and maintain professional relationships with
referral sources, including attending meetings and participating in
community events to promote hospice services. Document all patient
interactions, referrals, and outcomes in the hospices CRM system to
ensure accurate tracking and follow-up. Train and support new staff
members on the hospice liaison role, including best practices for
patient communication and referral source management. Participate
in interdisciplinary team meetings to provide updates on patient
referrals and to discuss strategies for overcoming barriers to
hospice care access. Qualifications: Bachelors degree in Marketing,
Healthcare Administration, or related field preferred. Minimum of 3
years of experience in healthcare marketing, specifically within
hospice or palliative care settings. Proven networking experience
across hospitals and healthcare systems. Strong communication,
presentation, and interpersonal skills. Ability to work
independently and as part of a team. Familiarity with EMR software
and marketing tools is a plus. Compassionate demeanor with a
