JOB DESCRIPTIONSummaryThis position will play vital role in the
Health Center's Primary Care program. The successful candidate will
be an advanced practice registered nurse who has acquired the
necessary training to provide primary healthcare services to
patients. The candidate will be responsible for caring for
patients, maintaining accurate and current patient records and
scheduling and administering follow-up appointments to patients as
needed. The candidate will also be able to develop an appropriate
treatment plan and provide effective management of patient care
with proper evaluation. The candidate can also function as an
educator to families, staff, and the community.Essential
Functions:

- Ensure patient information is always secure and

- Perform initial patient screenings and patient history and
physical examinations.
- Prescribe medications according to BACHC policy and to state
and federal regulations.
- Enter all patient information into the EMR.
- Analyze test data and determine the need for follow-up
appointments and further treatment options.
- Deliver quality care while maintaining all company metrics
pertaining to productivity.
- Formulate, execute, and review healthcare education delivered
to patients and their families.
- Participate in care team huddles, provider meetings, and
all-staff meetings.
- Perform all other related duties as assigned.Education
- Valid Nurse Practitioner and Registered Nurse License in the
state of California
- DEA certificate
- CPR CertificationSkills / Experience1. A keen interest in
helping the underserved population (e.g. the homeless
population).2. FQHC Experience a plus3. Familiarity with Quality
Measures a plus4. Able to be successfully credentialed by BACHC.5.
Strong communication and interpersonal skills6. Bilingual (English
and Spanish) a plusPhysical DemandsThis is a physical position that
will require standing on your feet, and walking. This would require
the ability sit, walk, bend or stand as necessary, and ability to
lift up to 25lbs.Other physical demands of the position include but

position. Days and hours of work are Monday through Saturday, hours
range from 7:30am-6:30pm with occasional approved overtime.TravelOn
