JOB DESCRIPTION SUMMARYThe Inpatient Licensed Vocational Nurse
works under the general direction of the Hospice Physician (or
Nurse Practitioner) and under the supervision of a Registered Nurse
(RN). - The Licensed Vocational Nurse (LVN) provides direct nursing
care and related assistance to patients and families at the end of
life. - The Inpatient LVN fully supports the Core Values and is
accountable for upholding the Standards of Behavior of VNA Health.
-ESSENTIAL JOB DUTIES AND RESPONSIBILITIESProvide hospice care to
terminally ill patients in the inpatient setting in collaboration
with the interdisciplinary team and other medical staff as
appropriate.Provide support for the patient's family.Observe
patients and report changes in condition to appropriate person
(e.g., RN).Maintain communication with patient and family including
education on pain/symptom management and psychosocial aspects of
the terminally ill.Must actively participate in the coordination of
all aspects of the patient's hospice care, in accordance with
current professional standards and practices.Participates in
ongoing interdisciplinary comprehensive assessments and assists in
developing and evaluating the Plan of Care.Attend and present
appropriate patient related information at interdisciplinary team
meetings. Assist the patient and family/caregiver and other team
members in providing continuity of care.Provide environmental
services as needed, maintain a safe and clean environment through
care of the patient's room and personal equipment and perform light
housekeeping duties.Provide nutritional services to patients as
needed, setting up and assisting with meal trays, feeding patient,
if necessary.Attend and participate in clinical education,
inservices, nursing staff meetings.Participate in Quality
Management and Utilization Review activities.Maintain necessary
records and reports in a timely manner.Applies working knowledge of
State, Federal, local and Accreditation regulations for the
delivery of hospice services.The above statements are only meant to

those stated in this description. All staff are expected to
participate as needed in variable shifts, including weekend
rotation per staffing needs and departmental holiday rotation.
-POSITION QUALIFICATIONSCurrent and active California Licensed
Vocational Nurse (LVN) license.Minimum one year experience
required. Experience working in an acute care and/or with
end-of-life patients highly desirable.Current CPR certification.IV
Certification preferred.Complies with accepted professional
standards and practice.Excellent observation, verbal and written
communication skills, problem solving skills, basic math
skills.Basic working knowledge and skill of computers (email)
required.Must demonstrate ability to communicate effectively, have
initiative, dependability and good judgment.Must be able to
collaboratively work as part of an interdisciplinary group, as well
as knowledge of pain and symptom management.Must be able to cope
with family, emotional stress and be tolerant of individual
lifestyles without judgment.Reliable transportation. -PHYSICAL
QUALIFICATIONSAbility to lift 50 lbs.Ability to push/pull 50
lbs.Ability to stoop, bend, and squat.Ability to walk up one flight
of stairs.Ability to walk one mile.Ability to sit for one hour,
