JOB DESCRIPTION SUMMARY The Evening Staff Registered Nurse plans,
organizes and directs home care services in accordance with
Medicare conditions of participation, Title 22, nursing standards
of care, VNA Health's policies and procedures and accrediting
bodies. This position is responsible for providing skilled nursing
visits to patients in late afternoon and after business hours,
Monday to Friday. This position also answers calls received after
business hours from patients, family and/or caregivers. This
position manages a patient caseload as assigned, and responds to
patients needing PRN visits. Respects and promotes the mission,
vision, and values of VNA Health. ESSENTIAL JOB DUTIES AND
Responsible for providing comprehensive care to home health
Completes an initial assessment of patient and family to determine
home health needs. Provides a complete physical assessment and
history of current and previous illness(es). Admits to appropriate
level of service/ care.
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and makes necessary revisions as patient
status and needs change.
Uses health assessment data to determine patient-centered goals and

plan that incorporates therapeutic, preventive, and rehabilitative
nursing actions including patient and the family in the planning

Communication Prepares clinical notes and updates to the primary
physician when necessary and at least every sixty days.
Communicates with the physician regarding the patient's needs and
reports changes in the patient's condition; obtains/receives
physician's orders as required.
Teaches the patient and family/caregiver self-care techniques as
appropriate, provides medication, diet and other instructions as
ordered by the physician. Recognizes and utilizes opportunities for
health counseling with patients and families/caregivers.
Provides a safe environment for the patient.
Assists the patient and family/caregiver and other team members in
providing continuity of care. Additional Duties Ensures that
arrangements for equipment and other necessary items and services

needs and departmental holiday rotation. POSITION
Current and active California RN license.
Bachelor's degree, preferred.
Minimum one year acute care experience required.
One year recent home health experience preferred.
Current CPR certification.
Licensed driver with insured automobile - in accordance with state
and/or organization requirements.
Responsible for supervising home health aides/ LVNs.
Excellent observation, verbal and written communication skills,
problem solving skills, basic math skills; nursing skills per
Ability to respond independently to life threatening
Ability to reason, deduce and carry out complex multiple step
processes. PHYSICAL REQUIREMENTS Ability to lift 50 lbs.
Ability to push/pull 50 lbs.
Ability to stoop, bend, and squat.
Ability to walk up one flight of stairs.
Ability to walk one mile.
Ability to sit for one hour, minimum.
